Population Health Academy Best Practice October Event
One of our ICB health inequality clinical leads and local GP, Dr Azhar Fareed, will be talking about the population health approach taken by his GP practice in Central Lancashire with a cohort of patients with diabetes/pre-diabetes. The cohort was a group of South Asian women and the approach took account of cultural needs to facilitate better engagement with diabetes prevention.
Everyone is welcome that has an interest in population health, reducing health inequalities and working to improve health outcomes and reduce inequity.
